Have you tried places like Slaters? My incredibly tall fella (taller than those mentioned before!) is a clothes nightmare (and picky with it as well) and it's about our only fall-back option sometimes! Failing that - I can lend you my brother with his knack of washing things so they end up with ridiculously long arms and shrunken other bits!Little miracle born April 2012, 33 weeks gestation and a little toughie!0

You can find a range of shirts which are slim and have long arms, but it's difficult to give a precise answer without knowing the neck size. You say he is medium build, so I have given some answers below for medium neck sizes.
15.5 inch neck - TM Lewin do Slim Fit and Fully fitted shirts with an arm length of 36 inches.
16.0 inch neck - Thomas Pink do a Slim Fit with an arm length of 36.3 inches
16.5, 17 and 17.5 inch neck - TM Lewin do Slim Fit shirts with an arm length of 38 inches.

For suits etc Next used to do a tailoring service that was not much more than standard to get a suit that fitted. Also for shirts etc try TM Lewin or Charles Tyrwhitt who both do longer sleeves. M&S do the 2" longer ones but even in our massive store they only have a small selection.
Other alternative that I used was get married, have kids then put on 2 stone so that you actually fit the clothes properly !! Wifey not too pleased her 44" chest, 32" waist has morphed into a telly tubby with 46" chest and 38" waist but my daughter says I'm good for hugs !0
