Starting the PPI process

I'm just about to start this process from the beginning. Only recently discovered i have PPI on my Lloyds TSB Credit Card...i have a couple of questions for those of you in the process or already done with it.
Firstly does anyone have a template letter for contacting the branch where the credit card was started/applied for? I can't seem to find one on the site.
Secondly, does anyone know if the PPI is provided by Lloyds TSB directly? If not who is their provider?

If it's a mis-selling complaint it doesn't matter who the provider of the insurance is but who sold it to you.
Use the Consumer Questionnaire and send it to Lloyds TSB or whoever sold you the policy. http://www.financial-ombudsman.org.uk/publications/technical_notes/ppi.html0 -

When you say Lloyds TSB do you mean the specific branch or the company as a whole?
