No paperwork for loan taken out in 2004.

Hi could anyone help me and just confirm I am on the right track.
I recently got hold of my Equifax report and on there was reference to a loan I took out in 2004 and finished paying back in 2007. (Knew I had the loan but actually thought that it was so old I could not do anything about it.)
I know that the loan had PPI but as I was self employed at the time I think now I probably was not covered.
I have looked at the template letter to get the credit agreement but unfortunately do not have the account number for this loan but from my credit report I can see how much my monthly payments were and what date the loan was taken out and what day the loan was repaid.
If I include this information will they be able to track my account?

Thats a bit of a leap. Whilst a small number of PPI plans do not cover the self employed, most do. How do you know yours didnt cover you?If I include this information will they be able to track my account?
