Cavity wall insulation

Back in October I had a survey to have free cavity wall insulation with Brirish Gas. I thought the surveyor was thoroughly knowledgeable and did a thorough job measuring the house.
After that followed a long frustrating process of waiting while they sourced some access equipment (otherwise known as a ladder?) because they would need to work above an extension.
They finally arrived today drilled some holes and then said that the cavity was not wide enough for their blown mineral wool because he had measured it at 45mm, and that would not allow it to spread properly. Apparently I would need bead filling, and British Gas don't/won't do that. Also the surveyor had missed the need for a safety vent to be fitted to the fire.
I'm now waiting for a re-survey. And then it will be the long waiting list, and then time off work again when they return to do the job.
I have to say I'm far from impressed, and since their engineers and surveyors seem to disagree, I wonder if any of tne experts here can advise.
What is the minimum cavity for the blown mineral fibre? Having agreed to do the job (I was paying a part of the 'free' cost because I'm slightly over the allowed size) are they obligated to complete with an alternative insulation? Can I find someone who can do the job efficiently, and send BG the bill?
