PPI reclaiming - 1st Letter Template... where?

hi all, where is the first letter template for ppi reclaiming? i have printed off the pdf questionnaire for the ombudsman (PPI-consumer-questionnaire.pdf) but can't find the first letter that should go to the bank... Thank you!

You dont have to write war and peace. The shorter and to the point the better. As long as your questionnaire has the mis-sale reasons in it then leave it to that.
