Be aware that in general, template letters have a lower success rate than personal letters. Especially if you use a template letter that is widely used and you include a load of reasons that are not applicable or can be proven to be wrong (you lack credibility if you use reasons that dont apply or can be proven to be a lie. In areas where there is no proof either way, a balance of probability decision is often made and the side that is most credible tends to get the nod).
If you use a template letter, then remove things that are not applicable. Personalise it in some way so it makes it relevant to you. The last thing you want is the complaints handler at the firm to look at your complaint letter and think "here we go again, another try-it-on".
