Help finding sleep chart (from another's post)

Hi. Wonder if anyone can help. Earlier today I read a post from a very distressed lady who felt she wanted to self-harm. Among the (mostly) very supportive replies, someone had posted a link to a CBT website, (something about work sheets) which included a work sheet about insomnia. I cannot now find the original post and hope someone can point me in the right direction. This would be really helpful as I think the sheet could be very useful to someone I care about.
