I have been reading this forum until my head aches

And I still am a bit confused

I took out a (secured) loan with Welcome in 2003, but paid it off in summer 2005. I still have the original credit agreement but I have no insurance policy number and no customer number. It shows the breakdown of PPI and the total of £2415.13. I know when I was sold that I was told by the lady that I needed to take out the insurance as a condition of the loan. I remember it so clearly because I thought it was so odd.
Is the next step filling out the FSA form, or do I need to write to the lender myself, and is there a template letter?
I promise I am not usually such a dunce

You need to ring Welcome and tell them you want to put a complaint in,they should send you a claim form.
The FSCS only deal with loans post jan14th 2005.0 -
