What to put in?

I am trying to claim PPI from Lloyds TSB credit card and i am having problem with this section : "What is the policy number of the payment protection insurance you are compaining about?"

What should i put in-Lloyds TSB credit account number ?

Also don`t know what is the best reason when answering this question :
"please tell us why you are now unhappy with the insurance?"

    I would leave this blank,you could put the credit card number,i doubt anyone will have the ppi policy number.

    Put why you dont want this insurance now,use the reasons for mis sell.

    Is this a good reason,what do you think?

    My English was poor when i signed for credit card (and still is ;o) ) so i signed everything what they offered me-
