Financial ombudsman forms?

I've downloaded the forms for my OH to claim back his PPI on his credit cards but I'm slightly confused by the forms, he's complaining by himself as they're his credit cards not joint but on page 4 of the form they are asking about partners details do we have to fill in my details on the form?

Some PPI can be joint. The forms are generic. So, if its not applicable, then mark the form so its clear it is not applicable.I am an Independent Financial Adviser (IFA).
