Financial ombudsman forms?

I've downloaded the forms for my OH to claim back his PPI on his credit cards but I'm slightly confused by the forms, he's complaining by himself as they're his credit cards not joint but on page 4 of the form they are asking about partners details do we have to fill in my details on the form?

Some PPI can be joint. The forms are generic. So, if its not applicable, then mark the form so its clear it is not applicable.
I am an Independent Financial Adviser (IFA). The comments I make are just my opinion and are for discussion purposes only. They are not financial advice and you should not treat them as such. If you feel an area discussed may be relevant to you, then please seek advice from an Independent Financial Adviser local to you.

Thanks we've always had separate credit cards so I'll leave my details off
