    look what i highlighted in red, yes you signed agreeing to the membership rules, even if you never read the rules by signing you accepted these.
    but if i were you i would contact the ICO (http://www.ico.gov.uk/complaints.aspx) as this would be a breach of the data protection act.

    If as you said, op signed and agreed to the rules, how would it still be breach of data protection?

    it would be if their was no opt out, but as OP said their was an opt out which they never used, so it is no longer a breach of the DPA

    if you signed the front page, only what is above your signature is valid contract, for the rear page to be valid contract this would require a signiture at the base, this is know as terminate, or end of contract, it is so nothing can be added in after the signature to the contract.
    So yes if you did not sign the back of this page at the end of the text, it is invalid, only what lies above your signature on the same side of the paper counts, with each sheet requiring a signature and the final requiring a signature and name.
