A2 income support form

hi everyone
i recieved an income support review form today,,just wondering is this something to do with being transfered to eas,ive been on income support about 6 years but never had one of these forms before,also it asks if im claiming i s due to illness then asks for details,do i fill in describing what im ill with?

I got one through the other week. It's just to make sure that you're getting the right amount of money I think and that your circumstances haven't changed. I sent mine off straight away and had a reply within the week saying that nothing had changed and they'd still be paying me the same amount.
As far as I'm aware it's nothing to do with ESA. If it was, I think you'd be sent an ESA50 form instead.2019 Wins
