Ending a relationship when getting joint Income Support

Hi, desperate for some advice to put my mind at ease.

Partner and I on the verge of splitting up, she claims income support for me. We will still be living together for a while until I can find somewhere else to live and afford to move. I am unable to work due to illness and was receiving income support before the joint claim.

Need to know what the process is for telling the DWP, will I have to make another claim or will they just transfer me straight back onto a single persons rate? How much of a delay will there be? I don't have much money to tide me over should I have to claim all over again and wait 6 weeks to be paid.
