Getting Trade Cards.. IR Form

People ask for "Copy of the Inland Revenue Self Employment form" to get trade cards.. I havent applied for this yet but will be doing so next week. Can I ask if anyone knows if they mean a copy of a receipt/letter back or do I need to make copies of the form before I send it? I don't want to post it off if I need to copy it in advance.
