Sore bottom crack.

No, it's not a cyst. It is defintiely just a long superficial crack in the skin. Inagine you pulled the skin apart just a bit too much, and it split.
Not cleaning themselves regularly enough - probably because it's sore. Might even be that they are experiencing difficulties in cleaning themselves after going to the toilet and the traces left are causing soreness.
Nylon undies won't help.
Washing 2 - 3 times a day with a gentle (possibly antibacterial) soap or hibiscrub, drying carefully and applying something along the Savlon/Germolene/Drapolene ilk for at least a week should help it heal quickly and keep it gone. Using baby wipes/moist toilet paper would also help - if they are extremely sore, I would recommend Johnsons Baby wipes above all others, as they are milder and softer than the other brands.
New, cotton undies. Wash them on 60 and not 30 to ensure all bacteria is nuked.
Oh, and it's something rather common with the larger build people out there. Probably because it's a bit more difficult to clean and dry every last bit when it's hard to reach. Applies to the less mobile of all sizes, though.
It's easier to say there is soreness than it is to admit that they can't clean themselves properly after going to the loo. Also means they are very unlikely to go to the GP about it due to sheer embarrassment.
None of this is a criticism, however.
Could they be needing perhaps a tool, available online or from Occupational Therapy to hold the paper? They might also be having problems getting in/out of a bath or shower, which can also be addressed and make them feel much better.

Without getting too personal, I suffer from quite dry skin that cracks easily, sounds similar I think. I use E45 wash in the shower which definitely helps in this area ! Makes the shower slippy though

I know someone this happened to but hers was as a result of overdoing it in a spinning class as she was up and down on the bike like a yo yo, landed funny and superficially ripped her butt!If you always do what you have always done, you will always get what you always got!0

I had cracks in my hand and skin problems on my abdomen that would not heal. Some lasted for years. I've tried all kinds of creams, multivitamins, washing, cleaning, etc.
Then I started taking simple chewable vitamin C tablets. They tasted really good, and I even started craving them. So I upped my intake until my craving was sated. For a few weeks, I was taking 500-1000 mg a day.
For the first time in years, I don't have weird spreading wounds on my abdomen, or cracks on my hands.0
