SEN - what are they looking for?

My DS is going through the assessment process for a Statement. I am now attempting to write our statement in support of the process and wondered what exactly are they looking for? What specifically are they looking for me to say?

I think I can say what difficulties he is encountering at the minute, but what historic information do they really need? If they are looking for 'when did you notice your child was having difficulties' the answer is I didn't!! Every year at his parents evening they said he was doing fine, and was around average for his year group. Suddenly in year 6 we got his first IEP and that was totally out of the blue. Hope this might explain why I might find writing the historic bit difficult.
