Got a copy of my M&S application today

It was for my M&S Credit Card.
I am planning on making a claim but I wonder if I would have any success?
The PPI box has been ticked but the tick does not look like the ticks I have made in other boxes. I have also signed to accept PPI however, the actual section where I should have signed and dated for the whole application is unsigned and undated.
I think therefore that I have signed in the wrong place and that perhaps a tick has been put in the box without my knowledge.
Should I just put the above as my explanation along with a copy of the application?
