Accessing Care Support & things like Think Banking

This is as much about the how-to as the financial side of this issue: MSE has just been a really helpful font of information and support so advice outside of the £ is very welcome.
Im essentially an unofficial carer for a young man with physical and mental health needs: he is on both DLA middle/higher and income support to live on and is in rented flat via HB. Presently a revised application for social housing is progressing as a result of his condition getting much worse over the past 6months.
My involvement with the situation has steadily increased over the years and I can now no longer both cope with the situation as a whole, but am also concerned that in order to stay in work myself I am going to need to move shortly and there is going to need to be some kind of help needed to replace.
Having had a brief look at the online resources I think the best step is to contact care direct first? The type of help needed is with things such as shopping, washing, routine cleaning, understanding letters/bills and managing the financial stuff: all in order to remain 'safe', as in it is demonstratable without any help this will all go very wrong. I realise many people have much worse care needs, but I am just trying to avoid a future bad situation from happening.
The Think Banking service is also something that needs to be explored, are there other such services?
