ESA Form - Where do I send it?

Hello!

Had a bit of confusion with the ESA form, I applied for contribution based when I should've applied for income related. They sent me another form and I've now filled it in, the problem is, it hasn't come with an envelope. I'm not sure where I should be sending the form back to?

Does anyone know? Would like to get it posted off today.

    The Benefits and Decision centre dealing with your case.
    There should be an address on the original letter to you.
    If not, then phone them on 0845 6088582, and ask where to send it.

    The new form should be treated as if it was received on the date of the original one, as long as you send it in promptly.

    Yeah, I spoke to a lovely man on the phone who explained I should put the same claim date as on the old form. I will post it to the address on the letter I have. Thank you.

    The address on the letter is often the processing centre the forms usually have to be returned to a postal sorting address. I would ring the 0845 number as suggested earlier, you don't want it getting lost in the system.
