Cancelling BT and joining Sky

I've been looking into cancelling BT at the end of my 18 month contract. I want to switch to Sky as for the same price I am paying BT I can also get an entertainment and sports package with Sky. I have also had a lot of issues with BT previously and quite simply just don't want to be their customer anymore.
I've searched on Google and it says that to cancel I need to send them a letter 1 month before the end of my contract stating I want to cancel - I'm more than happy to do this than to just rely on them recording my call saying I wish to cancel (which I will do) but I can't find an address to write to.
Also, does anyone know if they charge for the equipment (hub) when you cancel at the end of a contract? I'm happy to send it back if that would cancel any charge.
I was also trying to double check that I can get all Sky services where I live (which I could when I checked a few months ago) but now can't find the postcode checker thing on their website?
