travel insurance:basics/side plane trip

Prob a silly question: but when you take out travel insurance is it covering you in case of plane accidents too or does airline do that?
Reaon why I ask is that going to the States . Travel insurance for basic trip over there ok. We are getting plane to Grand Canyon rim then helicopter down to river for white water rafting trip. Travel insurance we have covers white water rafting.
BUT what about plane/helicopter trip. Do we need to arrange separate cover for this or will plane fare include insurance
Trip organisers included American insurance form in their documentaion but seems to be for standard holiday rather than this more risky side trip.
Any advise: anyone done anything similar?
