Anyone taken case to Health Ombudsman (NHS CHC)?

I can give more detail if people are interested but where we are now is that the report of the Independent Review Panel disgracefully misrepresented facts and traduced the family's testimony at the Panel. I have complained and have said that we are taking this to the Health Ombudsman.
Is there a protocol to follow, please? All I know at the moment is that we have a year to do this in.

After the local appeal then independent appeal the final step is the Health Service Ombudsman http://www.ombudsman.org.uk/. Write to them but make sure everything is well documented. Your testimony is worth diddly squat if you have nothing to back it up with.
Out of interest what recommendation did the MDT give in the DST? How many priorities, severes and highs? What grounds is the appeal on?0
