How do card scammers get your details?

I have several accounts, one of them (Barclays) is used for big bills, I never use the card apart from the odd yearly bill. It'd been about 8 months since I last used it and I did a phone payment for B&C insurance to Shiela's Wheels, then 2 weeks later get a call from the bank fraud dept. that it'd been used online for a few unusual things. Got it cancelled and new one sent out, all dealt with easily, thought that was the end of it and the dodgy bloke in the call centre swiped my card number!
Anyway, this week one of my other accounts (Lloyds) phoned with exactly the same thing, as before I phoned them (direct) and basically got the same information, online purchases that were unusual, read out to me over the phone and I didn't recognise them etc. card cancelled and new one on the way. Difference is this account is used A LOT! Cashpoints, C&P in shops, online, etc.
So now I'm paranoid, am I being stalked or something? I don't have paper statements and am pretty good with security for online banking. So is it likely to just be a coincidence that I've never been hacked before but now had it twice in as many weeks? Do fraudsters work in call centres? Is the Lloyds one likely to be online or a double swipe in a shop or something? I always cover my pin, so must just be the info on the card they've gotten hold of (numbers etc.) for buying online?

NEVER, EVER trust a inbound call when they say they are from THE BANK fraud department.
Get their name and extension number, and call the bank back on a number THAT YOU KNOW. e.g. The number that will appear on the back of your card normally.
YOU CANNOT TRUST people are who they say they are on the phone.0 -
They tend to buy them from dodgy call centre reps, or from the internet.
Do a search on "where can I buy track 1 and 2" and you will be amazed.0 -

That's why I said I phoned them direct. Half expecting them to say they didn't phone but both times said yes there is activity and put me through to fraud.
