Disability Premiums/Benefits etc

I have been registered Disabled since 2002, I recieved DLA(higher Care) (Lower Mobility) and Incapacity. Recently I moved out of parents and I now get income support of 55 pound a week.

Should I be getting any disability premiums aswell? Or in the past when I was at my parents?

Does anyone Know what forms I need to apply for Enhanced Disability Premium, Severe disability premium and the disability premium?

    you need to notify your bdc,keep in mind you will only get sdp if no one gets carers allowance for you and you live alone
