What are the various factors that determines the cost?

Depends on how much the train operator wants to charge and also where you are sitting on the train (it is free in first class on East Midlands Trains for example).0

Only market forces. Even in Standard class there can be promotions where the entire train has free WiFi on an ad-hoc basis - just listen for the announcements and you might be lucky.0
