debenhams store card account number

Hi ,this is my first post so be gentle with me!
I am in the process of writing to debenhams about my store card to ask for my credit agreement to check if I had ppi. I paid the card off a couple of years ago and don't have the account number anymore. Using the template on this site it suggests putting the account number in but obviously I don't have it!!
Any suggestions on the best thing to do? Any advice would be appreciated thanks!!:)

Leave it blank.
Ring them up and ask for it.
See if you can find any old statements / letters.
See if a reference number appears on your list of direct debits or on your bank statements.0 -
