Pizza Express Giftcard - Error or Scam ?

In July, I attempted to use an unused Pizza Express gift-card at my local restaurant only to be informed there was no balance remaining on the card. Obviously, this was quite embarrassing.
After speaking with their Customer Service Department I was informed that the card had been used several months earlier at a restaurant 100 miles away that I have never visited.
Over the past 5 weeks I have repeatedly contacted Pizza Express for an update regarding this issue but have found their level of customer service to be poor. I have not yet been provided with an explanation or a refund.
Essentially, both the gift provider and myself have lost money due to this episode.
My question is - Has anyone else been subjected to this by Pizza Express, or is this a one-off card duplication error on their part ?
