Producing Forms in Adobe

I'm trying to produce a 'form' / questionnaire as a PDF.
I want to be able to include text entry boxes/drop downs, selections etc - but protect the form (and generally have it look "clean").
I can produce what I want in Word, but my preference is to have it as a PDF so those without word can open it etc.
I am using Adobe 6 Pro - but can't seem to create a "Form".
Can I do it, or do I need to buy something else from the Adobe family to help me do this?
Is an alternative to do it as HTML - and then add a button to email the results to me? Any ideas on how to do this?

The form fields would be a feature of MSWord so I don't think you can do a pdf version.

A simple way is to create the HTML form with a mailto action on the submit button. A nice example here at www.w3schools.com.0 -

In Adobe 5, you have to display the Tag Viewer (I only have access at work, and cannot check - sorry), declare the text defining your input boxes, and then mark out the space for input areas. These can then be converted into list boxes, validated data boxes, date boxes and so on via properties.
It can be done in Adobe - but you will have to resort to the built in help as my memory is hazy at best and can only remember the general principles.
Go-on, hit the F1 button, you know it makes sense.
