Does anyone have a blank copy of the application from saved somewhere that they could send me/upload?
My dad tried to open one of these accounts the other day but wasn't able to print out the application form.
He emailed them to query this but by the time they replied it had gone from the website and now he's got an email from West Bromwich reminding him to send in his signed application form!

I suppose your dad could try sending a cheque with a copy of the email to confirm he had applied. Does he have any other account (eg their regular saver) with West Bromwich? Is there any other form to download which would include their demutualisation waiver?
Added: Just read the email I received and see that the last day for getting funds to them is tomorrow, 30 August, so unless you contacted them on Saturday to get something emailed to you and can get to a branch to deliver it there doesn't seem much chance of getting this account.0
