DLA change of circumstances

can he download a change of circumstances form or does he need to phone them?

I'm not sure there's a form specifically for change of circumstances. When I've had changes I've always phoned them and had my comments added to my account. The advisors told me that they'd send me a reassessment form if the change is considered to affect my DLA or I could request a reassessment if I thought my change of circumstances increased my needs.

it is a DLA434 form.

they don't have it electronically.

you have to phone and they send out a paper form to you with a reply slip

the forms were updated in april 2011 and have not been released electronically

They were updated again in January 2013. As far as i'm aware they are not available in an electronic format.
