Not necessarily. One of the most common reasons for erroneous transfers is address' getting mixed up. For instance, you live at number 16 on your street and the person at number 60 tries to change supplier. Due to mis-communication, the agent types the wrong house number and your supply gets taken over instead. THe same thing can happen with getting the postcode slightly wrong so that the wrong property switches. Sometimes it can also be that the industry databse has two supplies mixed up, so even though the agent inputs all of the details correctly, the wrong property still changes supply.
