I agree with this though it is possible to put details about his worst days as well as his milder days. My health needs are highly variable so to answer almost every question relevant to me I wrote about my worst days and guessimated (using a diary) how often I get really bad days then wrote about my better days and how often they occur. I wrote 'see additional information' on every question as there was no way this could fit into the box. Don't be afraid to use extra paper, just make sure the pages are clearly labelled so the decision makers know which information relates to each question.kingfisherblue wrote: »If you treat every day as if it is his worst day, you are lying and can be prosecuted. I wanted to provide information on both my better and worst days so the DM got an accurate picture and I reduced my paranoia of being awarded something I wasn't entitled to and getting done for fraud.
It seemed to work as I was LRC/LRM (I was only expecting LRC) on the basis of my form and a report from my consultant.

i used the NAS dla supplementary information form - available from their website. really helpful.
would echo what mrs manda has said about stating 'see extra info' on all the answer boxes. i have been helped by a local specialist charity (no NAS around here) and they have reported that they always do this with claims.0 -
