Cloakroom into wetroom

Hi, we are thinking of moving house and would ideally like somewhere with an ensuite or extra shower room. Trouble is a lot of the properties in our area are older and therefore tend to have either a downstairs cloakroom or a toilet upstairs (usually no sink). Does anyone know if it is possible to convert rooms like this into a wet room. I would have thought that if it was possible then one of the many houses I've looked at would have done it, but would love to be proved wrong!!
