I cannot answer for esure in particular but in general insurers will "slip it in" in the legal talk they give you (which most customers do zone out during) over the phone or in the important information for those that buy online (with our site a customer cannot buy without clicking "I agree" to the page and a half with the important information statements and even though the page is that long the average time spent on it by customers is under 2.5 seconds).
For us, it is also stated in the T&Cs of your policybook and in the important information leaflet that accompanies the renewal invite - plus the invite says if they are on manual or auto renewal. Even though we tell our customers at the time of purchase, at the time we send their docs out and at the time of renewal we still get a percentage of people saying they hadnt been told.
As with all technology, things can go wrong - some will also say technology has gone wrong and give someone the benefit of the doubt when in fact the technology has worked perfectly and they just dont want to admit that their employee made an error.
