HELP!!! 3yrs trying for a baby

I have been trying for a baby since Aug 2008. Then I found out I had a varicocele in Dec 2010. I gave 2 sperm samples for analysis and the doctor said they were not looking good and recommend varicocele embolization. After the varicocele embolization (May 2011), I gave another sperm sample for analysis (results shown below). Still no luck with a baby. Now the doctor recommended that I get my wife checked out, which she is in the process of doing.
Should the doctor have referred me to the fertility clinic, by the results of my last sperm sample? I am 27 and my wife is 30. We don’t drink or smoke and are fairly healthy. I take multivitamins. Can we do anything else to increase our chances?
Sperm Sample After Varicocele Embolization
Volume - 3.2 - ml
Count per ml - 15 - x10^6 /ml
Total Sperm No - 48 - x10^6 /ml per ejaculate
Motile Sperm - 40 - % approx
Mean Progressive Motility - 1 - 0=None, 1=Poor, 2=Good, 3=Excellent
Opacity - N - D=Decreased, N=Normal, I=Increased
Viscosity - N - D=Decreased, N=Normal, I=Increased
Abnormal Forms - 90 - % approx
