is it best to reclaim ppi back from lloyds by letter or over the phone with lloyds it self???

Give Lloyds a ring and they will take your details over the phone.

There are a few ways you can do this, as amersall says you can ring, or you can download the reclaim questionnaire if you prefer, as shown on their website, you can do this by posting by hardcopy to the address as also shown on their website or email them here.....
