Code for Form and SQL database

I know a little about creating websites and HTML/PHP and I am looking for a piece of code that is already written that I can modify to suit my needs.

So I have a set of questions consisting of Radio Buttons, Drop Down Boxes, Text Fields, etc and I am looking to integrate this into an SQL database such that users can input their information and then at a later date retrieve their information and modify the data. This would be need to be via a User Name and Password.

Is there anything out there that all ready exists that I could use?

    I think most people would recommend a CMS like Joomla/Wordpress/Drupal, but these all seem too much for what you need, unless of course you're looking at expanding the website in the future.

    If you're looking to learn PHP/MySQL and create something of your own from scratch, then tizag.com is a very good site to learn from.

    I dont recommend it unless you really know what your doing, its fine to learn, but please dont go learning on a live website with REAL peoples customer data, passwords etc.

    Far to easy to leave way to many security loopholes and have the data accessible to people you really dont want getting hold of the data.

    Good for you wanting to learn to program, but be aware database architecture and web integration is actually a large subject in itself - then when you add in the vulnerability of SQL Injection (the most common way websites get hacked) and you may be better off using a CMS platform (joomla/drupal?) or even something like surveymonkey or wufoo (both .com) which not only gets the data into a database but allows you some flexibility with reporting too.
