If you are part way through a policy they will probably increase the premiums and add an admin fee - your mileage payments are to cover costs such as this.

When it comes to renewal shop around and you may get it added for no or little extra cost, but it will depend on how many business miles you do.

How much does £193 represent as a percentage of your annual premium?

Get new quotes online with your existing insurer, one without business use, and one with. Then you can see if there really is a difference.

If you ever need to carry passengers as part of your job - this is a material fact and should be declared.

It will depend what class of business you need to add.

If it is simply going to multiple work locations and your not carrying goods or selling then Class 1 business use should be sufficient and it is normally free. Class 2 is the same as class 1 but adds the same cover for named drivers and Class 3 allows you to carry goods or do door to door sales etc.

None of these would allow you to carry passengers for hire or reward, if you are carrying your patients around I would certainly check with the insurers in question if this would count as hire & reward, if it does then you move into a whole new field of basically needing taxi insurance
