o2 mobile rip-off

It looks like o2 are creaming off credit balances. If an account is closed and a balance is overpaid. o2 advise you by text that you need do nothing; they will take care of it and refund. Of course they don't do anything, just let the cash sit there and hope you will forget about. When you contact them they suggest it is your fault for failing to fill out a refund form [which they claim is absoulutely neccesary to facilitate a refund].
They are being obstructive and underhand in their dealings, fobbing you off in the hope you forget or get tired of an obstructive attitude.
My question is how many accounts are in credit and cash not returned?
