Questions asked at DLA EMP assessment/report?

Hello,

Due to have an EMP visit to do a report for DLA next week.

What kind of questions do they ask?
Are there set questions they ask/follow a form?
Do they do a physical examination?
Do they need a tour of the house to see adaptions?
Do they need to see hospital letters, reports, scans etc?

Any advice would be welcomed.
