Vat exemption for disability

What does one have to do to be able to freely claim Vat exemption because of disability? I have made a few purchases from CosyFeet for shoes, and always had Vat exemption, but if I wanted to buy shoes from a high street store, can I ask, claim or have to show proof?

I don't think shoes from a shoe shop with count.
You can only get VAT exemption on items that are of practical help to you because of your illness or disability. This link will give you more details
