ESA Medical Questionaire

hi,just looking for some advice,i posted my esa medical questionaire 4 weeks ago, today i was contacted by the dss as to why they have not recieved it as yet,i told them when i posted it and the lady said that if its not been recieved by 31 may my benefits will be affected,i told her again that id posted it roughly a month ago so they should have recieved it but she said it was down to me to prove it and that it is now to late for new forms to be sent out and for them to recieve the new forms back by the 31st may and that the likelyhood is that my benefits will be affected,i dont no what to do as my esa is due on tuesday and if they still say they have not recieve the forms by tuesday can they simply not issue my payment and what will i do regards money for food etc,tia

Stop posting here for the moment and immediately fill another two out, hand deliver one today [ when you take it in ask for a stamped receipt ] and send the other recorded 1st Class delivery.

hi richie,thanks for the reply ,however im unable for some reason to fill the forms out online ans then save and print them as i keep getting error messages when i try to access it,as i say ive told them the forms were posted and all i asked was can they resend me new ones to fill out but the lady i spoke to kept stating the date required by,surely if ive posted them they can send new ones and extend the date to recieve the new forms if they would simply agree to send them

- get Acrobat v8.1.2. or later .. .. here is v 10 then you will be able to deal with it online.

I'll bet my house you will not be allowed to extend it, their job is to get 30% of the population off the benefit. Given half a chance and they will take it, in your case my friend you are handing it to them a 100% on a plate, their puter will already be pre-programmed to stop you money from the said date.

If you can't do it yourself .. .. get someone else to help you, but get both forms done and one of them hand delivered before close of business Thursday, it's a bank holiday weekend.

Stop hoping for a reprieve, its a forever rehearsal for a show that will never happen. you must start doing it now.

Incidentally, unless you can prove that it was posted and that it was received by the DWP, they will stop your benefit as from the date they told you that it had to be received by. Many, many try this con out every week - swearing blind it was sent off when they realise that their money has been stopped.

hi,just wondering how to get the address to post the esa forms to atos,can i simply phone the dss and will they tell me the address,and thanks to richie for helping me to access the forms online,
