I am in the process of filling in the form for dla. On the part you have to list your medication is that just the medication you are taking for what your claiming or alledications, hope this makes sense?

I think you're supposed to list everything, all medications, all conditions, all care/mobility needs.0

List all the prescribed medications. You don't apply for DLA because you have a specific condition (except is special circumstances), you apply because you have care and/or mobility needs which have lasted 3 months and are expected to last at least a further 6 months. Therefore the more information you can give about your health status and the effect it has on your care/mobility needs the better.0

Thank you. Also the form asks for assessments and reports (which I don't have many of, all with my gp I would imagine) should
I include appt letters for all my conditions?0 -
Include as much evidence as you can - your GP should be able to give you copies of consultants reports, etc.
By the way, it would probably really help your claim if you were to get someone from CAB, DIAL or a Welfare Rights Officer to help you fill in the form.0 -

Ask your GP for copies of any relevant reports recieved, or ask your consultants to send you copies of what they sent to your GP,and a good tip is to include some further info, from yourself, about how your conditions affect you.
The form is ok, but an extra typed sheet or two giving fuller information is a good idea.
This site gives some help on filling out the forms:

You can tell a lot about a woman by her hands..........for instance, if they are placed around your throat, she's probably slightly upset.
