Social Services, is there a form for this?

I've moved house! (very excited, thought it would never happen).
I'm now a resident of Glasgow City and so I am entitled to a needs assessment from their social services department.
I'm only really looking for a fall alarm, the rest I can handle between DLA, my sister (currently my carer) and access to work.
Now on the Glasgow website it says you should call into a social work office or call them but is there a form I could use instead? I mean I live round the corner from one of the social work offices but I hate speaking face to face with official people and would much rather have a form as that is much easier for me to articulate my point.

Hi there,
I know what you mean about dealing with people. I'm my husband's full time carer so a lot of things are up to me to sort out but being partially death I find dealing with people face to face or on the phone difficult so I like to try and do things via the internet. I'm not in Glasgow but if I need to contact SS for my hubby I email them or use the online form on the council's website.

you can refer yourself for the telecare service
(at the moment it was free but they keep threatening to impose a charge)
