section 36 on under 16s DLA form

i had help to fill out the form and the lady at cab told me to fill in section 36, she may have said i have to get it filled in, not sure tho. the bit that asks someone who knows and sees the child on a regular basis.
surely i dont fill it in as i have given all the other info
so if i dont fill it in who is the best person to fill it in
his sen at school knows him well or should my OH do it or someone completly different

in my expereince of volunteering for a charity helping disabled children and their families with claims for DLA - the DWP do EXPECT some form of "professional" person to complete that part of the form - legally they are NOT allowed to state it must be a medical person, but more often than not if you don't get a medical person to sign, your claim will not succeed.
If you have input from another agency for your child, such as Child & adolescent Mental Health service, or a physiotherapist/occupational therapist, then they would be ideal to complete that section - however, if you don't then your sons SEN teacher would be an ideal person to complete it- but remember, your son gets the award for how the illness/disability affects his ability to deal with his personal care needs and for any extra care needs or supervision he requires to avoid injuring himself, so the SEN teacher MUST put in the extra care that he receives at school - otherwise simply putting a diagnosis won't be sufficient.

thanks, it will have to be the sens teacher as he has only just been refered to occupational therapy and we dont see them till june and the date the form has to be back by is next week0
