Name Change Deed Poll

I changed my Christian name 15 years ago using a statutory declaration signed by two witnesses. I informed all the relevant organisations and have documents like passport, driving license et al in my new name.
However, I recently go a letter about the NHS summary care record in which I discovered my old name was being used. After contacting them to correct it they tell me that my GP clinic has had my current name down as my 'preferred name' for the last 15 years and that I need a deed poll attained through a solicitor for them to be able to change my name on my NHS record. I have lost my original statutory declaration but according to the GP's secretary it wouldn't be accepted anyway. They say it was originally not changed because I did not show a 'deed poll' and only had a 'non-legal' declaration!
I am basically now looking around for some sort of legal looking document that will convince them that I am entitled to change my name and wondered if the following website is legit:

I use a statuary declaration, it's valid if if witnessed by a solicitor, which is usually about £10 fee. Chances are the GP's solicitor didnt know what it was and said that it had to be Deed Poll! Just print off the stat dec. again and book an appointment with the solicitor! x0
