I have been paying this since I got the card and now have closed the account due to the interest change, I'm just wondering, can I claim this back without having to fill in the form that they send me (includes a form a doctor has to sign) etc

Are you trying to claim the sickness benefit provided by the policy?
If so, you need to provide evidence of illness. Doesn't sound unreasonable to me.0
