Broken concrete manhole cover

I've got a concrete manhole cover on my driveway that's fallen through and I'm struggling to get it fixed.
Can anyone help? It looks quite hard to remove, does anyone know how I'll be able to and where I can get another from?
I did try ringing the water board but they said it's my problem as it's on my land.

It's quite easy to fix actually.
You will need a complete replacement manhole cover and frame.
Remove the cover then chip out the frame using a bolster chisel and mallet. Clean off the mortar around the bricks/blocks underneath where the frame was.
Using a bag of mortar (you can get ready mix from DIY sheds and builders merchants) mix this up with some water and make a new bed of mortar.
Set the new frame on this bed making sure that it will hold the cover level with the driveway. Do not place the cover on at this point in time. Float off any voids and level with the driveway using more mortar but make sure you don't put mortar anywhere that will stop the cover sitting properly on the new frame.
