Backpacker / Volunteer Insurance Costa Rica

I am trying to find the best deal for my son. He is off to Costa Rica to do volunteer work working with animals in a rescue centre. The range of animals is pretty diverse.
He has a placement for two months from May to end of June, but is hoping to get an extension or even a differet placement within Costa Rica.
I know we need to have repatriation (high on my list) along with full medical care, but trying to pick the best package is prooving difficult.
Not helped as he is so vague about dates. We have flights booked 25th April out and return 26th August, but the return date is flexible depending on what he can achieve when he is out there.
Any advice on choosing a good policy and getting it right would be much appreciated.
