HSBC Plus World Wide Insurance (Aviva)

Hi,
I have World Wide Travel Insurance through HSBC provided by Aviva.
Recently whilst away I had to claim for medical costs. All done legit, called them and got authorisation to go to Hospital etc.
Returned to UK and paid excess of £50 and forwarded bill which I was invoiced from Hospital as requested by Aviva.
I have had my prescription charges refunded but they are now saying that I have to settle the bill direct with the Hospital and then claim it back from Aviva.
I have a feeling because on the bill it says a discount of $200 has been applied if I pay the Hospital direct, but if the Insurance pays this discount does not stand (total $406.00).
I have told Aviva I do not have the funds to pay this and that they are being tight and cheap because they do not want to pay the extra $200.
I have never heard of having to settle this with the Hospital and then claim it back? Any advice?
I thought the whole point of Travel Insurance was that they would settle the bill direct considering they advised me to go to that Hospital as they have 'arrangements' with them.
